The alcohol flush is not an allergy. It is a traffic jam.

One drink in, and some people turn visibly red: warm face, faster heartbeat, sometimes a headache or nausea. It gets called an alcohol allergy, but it is nothing of the sort. It is a metabolic traffic jam, and the genetics behind it are unusually clear.

Two enzymes, one bottleneck

Your body clears alcohol in two steps. First an enzyme called ADH converts alcohol into acetaldehyde, a genuinely nasty intermediate. Then a second enzyme, ALDH2, converts acetaldehyde into harmless acetate. The flush happens when step two cannot keep up with step one and acetaldehyde pools in the blood.

The main culprit is a variant in ALDH2 called ALDH2*2 (marker rs671). The A version produces an enzyme with drastically reduced activity: one copy strongly slows acetaldehyde clearance, and two copies nearly eliminate it. Carriers flush after small amounts of alcohol because the cleanup crew is understaffed. The variant is common in East Asian populations, where roughly a third of people carry at least one copy, and rare elsewhere, which is why the flush is sometimes called Asian glow.

A second gene can pile onto the same jam from the other side. A common ADH1B variant (marker rs1229984) speeds up step one, producing acetaldehyde faster. Someone with fast production and slow cleanup gets the strongest reaction of all.

Why the flush deserves respect

Here is the part worth taking seriously. Acetaldehyde is not just the source of an embarrassing glow, it is a toxic, DNA-damaging compound, and people who flush yet drink heavily anyway are marinating in more of it than non-flushers at the same intake. Population studies consistently associate heavy drinking in ALDH2*2 carriers with worse alcohol-related health outcomes.

Which leads to one blunt piece of advice: do not mask the flush. Antihistamines can suppress the redness, and that is the single worst trick on the internet, because it silences the alarm while the acetaldehyde stays. The flush is honest biological feedback. Interestingly, the same variants are also associated with drinking less and with lower rates of alcohol dependence, for the obvious reason: when alcohol feels bad quickly, people tend not to overdo it.

What to do with your genotype

If you carry the slow-cleanup variant, the practical playbook is short: treat the flush as your real limit rather than a social inconvenience, favor lower-alcohol options and slower pacing when you do drink, and skip the folk remedies. If you do not carry it and you still flush, something else is going on (some medications and skin conditions mimic it), which is worth knowing too.
